3P loop, TEAM & risk management steps
Decision loop (repeats continuously):
- Perceive hazards
- Process into risks
- Perform chosen control
- Evaluate result
Risk: future impact of a hazard not controlled or eliminated. Three factors:
| Factor | Measures |
|---|---|
| Exposure | Number of people or resources affected |
| Severity | Extent of possible loss |
| Probability | Likelihood hazard causes a loss |
TEAM checklist (PHAK ch.2), applied to each hazard in the perform step. One hazard can take several controls.
| Control | Meaning | Example |
|---|---|---|
| Transfer | Hand decision to another | Consult chief flight instructor |
| Eliminate | Cancel or avoid hazard entirely | Cancel trip |
| Accept | Benefits of the risk outweigh costs | |
| Mitigate | Reduce residual risk | Pick suitable alternate |
Six risk management steps (start at identify hazards):
- Identify hazards
- Assess risks
- Analyze controls
- Make control decisions
- Use controls
- Monitor results
Two practical tools:
- Personal minimums checklist: set before the trip; see
aeronautics/personal-minimums. - Post-flight analysis: after securing aircraft, review flight objectively. Learn from mistakes before next flight.
